As you know, the US Federal Government and most states have laws that protect personal information from public disclosure. Such laws are stringent and, at the federal level, include the Health Insurance Portability and Accountability Act of 1996 in the United States. Most of these laws forbid the public disclosure of personal information about a person or patient, without their consent. LifeWallet makes every effort to secure the information that you upload to this site, but can provide no absolute guaranty against unauthorized access by third persons over whom we have no control. We also do not control the persons to whom you provide or share your site passwords and access information. Therefore, it is your responsibility to control access to your passwords and login information.
Any health information that you submit to our site is owned by you or your patient, as applicable. Notwithstanding, you and/or your patient allow us to use any such data on a de-identified basis such as, for example, understanding your likes and dislikes, your search and reading tendencies and other matters. Such data also includes information about your health and wellness, such as medications that you are on, your age, general health, specific conditions, geographic location, and medical history. You agree that LifeWallet may sell your de-identified data to third parties for use in reports, studies, analytics and statistics. The term “de-identify” means that your data has been stripped of information that enables a third party from identifying you or your patients. If you terminate your relationship with LifeWallet, you will then be entitled to one (1) copy of your personal data on the site. You may request it in writing to firstname.lastname@example.org. Such requests may not be made later than 30 days’ post- termination. Should your data exceed specified storage sizes, you may be charged for a downloadable copy of your data. However, you agree that LifeWallet may retain your data for the purposes described above.
The security of your information (or your patient’s) is important to us. When you enter information (including health information) into our site, we encrypt its transmission through the use of SSL connections (Secure Socket Layer technology). We will retain your information (or your patient’s) for as long as your account is active, or it is used to provide services to you (or your patient). At any time, you may remove your information (or your patient’s), or instruct us to remove it, but you should be aware that it may not be possible to remove every record that you have provided from our servers. LifeWallet tools that collect and store data allow you to correct, update or review information you have submitted, by going back to the specific tool, logging-in and making the desired changes. We will also retain your data, if necessary, to comply with legal obligations, resolve disputes and enforce our agreements.
If you choose to register or update your member profile, or access certain functionality on the LifeWallet site, you may be required to submit Personal Information. “Personal Information” means information that we can use to identify or contact you, such as your name, address, telephone number or email address. Personal Information also includes information about you such as your age, sex and medical history, among other things. Some of our interactive tools and services may request that you submit your health information, which would also be considered Personal Information. You must ensure the accuracy of the Personal Information that you submit to LifeWallet. Inaccurate information will also affect your experience using the LifeWallet site, and our ability to contact you, if needed.
At the time of registration, and at other times, you may be asked provide us with Personal Information. If you do, we will automatically register you to receive recurring informational/promotional newsletters, advertisements and emails, such as those pertaining to a specific health conditions or topics. If you no longer wish to receive such communications, you may unsubscribe, or change your communications preferences in your web browser.
LifeWallet features community and public forums where users (with similar medical conditions or health issues), may share information, or post questions for experts to answer. We also offer online discussions moderated by physicians and experts. Our communities are open to the public, and should not be considered private. Any information (including Personal Information, about you or your patient) that you share in an online community area including a chat room, blog or online discussion, is open to the public and is not private. You should think carefully before posting any Personal Information in any public forum. What you post can be seen, disclosed to or collected by third parties, and may be used by others in ways that we cannot control or predict, and that could be used improperly to contact you for unauthorized purposes. As with any public forum on any site, the information you post may also show up in third-party search engines. If you mistakenly post Personal Information in our community areas, and would like it removed, you can send us an email to request that we remove it. In some cases, we may not be able to remove your Personal Information.
If you do not want your Personal Information used by LifeWallet, you should not register as a member or use this site. You may correct, update or review Personal Information that you have previously submitted, by going back to the specific site, logging-in, and making the desired change. You may also update any Personal Information that you have submitted by contacting us. If you have registered, and desire to delete any Personal Information that you have provided, please contact us. Upon your request, we will delete your Personal Information from our active databases, but it will remain on our inactive databases. Remember, however, that we may continue to use your Personal Information on a de-identified basis as described above. If you decide not to receive personalized email or secure electronic messages, you may "Opt-out" by changing your email preferences, or by following the unsubscribe instructions included in each promotional email or newsletter. You may also change the user settings in the site.
Personal Information and Non-Personal Information that LifeWallet collects, may be combined with other information that it collects for research and measurement purposes. This information may include your/your patient’s age, sex, illnesses and medications, and information about you/them such as demographics, geographics, personal interests, purchase activity and other information. This Information will always be de-identified, and will likely be aggregated, and may be sold to third parties. In all instances, your Personal Information will not be traceable back to you, without your consent.
We are committed to protecting the privacy of children. The LifeWallet site is not designed or intended to attract children under the age of 13. The LifeWallet site does not collect Personal Information from any person we know is under the age of 13. A parent or guardian is solely responsible for providing supervision of a minor's use of the LifeWallet site. The parent or guardian assumes full responsibility for ensuring that the registration information is kept secure, and that the information submitted is accurate. LifeWallet will not permit users under the age of 13 to register to use the LifeWallet site. Furthermore, LifeWallet will not permit children between the ages of 13 and 18 to purchase goods or services through the LifeWallet site.
